The Patient Medication Profile (PMP) is more than just a printed list of all your medicines. The profile contains specific information about each medicine such as the active ingredients, dose, what it is for and how to use it. It will also contain a picture of the medicine for easy identification in most cases.

A home medication review can be arranged by asking a GP for a referral. They are particularly helpful for people on multiple medications, those who may be confused or concerned about their medicines or anyone who has just spent time in hospital. It is a free service funded by Medicare.
Your GP requests the review by writing a referral to the pharmacist who then organises a Chemsave pharmacists to visit your home (preferably) to discuss the current medications you are taking. The pharmacist is looking to ensure that medications are being taken regularly and correctly, to check on any out of date medication, and if there are any contraindications with any of your prescribed or general medicines. The pharmacist will also show you how to take and store your medicines correctly and will provide education about any issues that the patient may have in relation to their medication.
This consultation will last between 30 and 60 minutes and the pharmacist will write a report for your GP after the consultation. Your GP will discuss any recommendations with you and may make appropriate changes to your medication regimen if required. Webster-pak is a safe and simple medication pack. It sets out all the tablets and capsules that need to be taken at each particular time of the day, for each day of the week. This makes it so much easier to manage medication especially for people taking many medicines where anyone can easily forget.
The pharmacist takes the doctor’s prescriptions and dispenses the medication into a blister pack. This pack is securely sealed so the medication is protected and there is no chance of spilling or mixing things up. The pack lists all the contents plus other medications taken, providing vital information for hospital or ambulance staff in the case of emergency.
When it is time to take the medication, the person or carer simply pushes out the contents of the blister through the foil backing for the correct time on the correct day. Thanks to Webster-pak’s distinctive visual cues, you can see at a glance whether someone is taking medication correctly or not. With regular bottles or packs it is almost impossible, and you wouldn’t know if someone was having a problem and may need some extra help. The Entrance Chemsave Pharmacy is a licensed disposal outlet for any unwanted or out-of-date medicines. We will accept out-of-date or unwanted medication and dispose of it safely and in the correct method.
We have a special disposal bin on the premises for the safe disposal of sharps/needles and syringes usually for the convenience of diabetics. Please note that the contents MUST be contained in a sealed plastic container for safety and hygiene purposes before we can accept these.
This service does NOT include the supply of replacement sharps containers or needles.
We also stock special plastic puncture resistant sharps collector containers of various sizes for safe storage and disposal of sharps and needles.
